Testing Accommodations for Students with Disa

These policies and procedures are used when tests are administered in Educational Services for Students (ESS). The staff of Disability Resources in ESS determines testing accommodations. Students will present a memo to their professor that will indicate the needed accommodation(s). The professor and the student need to work out a method for providing the needed accommodation(s).

Faculty Authorization Form

Students requesting test accommodations that will be administered in ESS will ask each professor to complete a faculty authorization form that must be turned in to ESS prior to any testing. Students should participate with their faculty member in completing this form so that all parties understand the arrangements. This form establishes the agreement between ESS and the professor to administer tests for their students. ESS insists that students taking tests in ESS uphold the University’s Code of Ethics. All infractions will be immediately reported to the professor.

Test Reservations

After speaking with their professors, students make reservations online. Tests will be administered Monday through Friday from 8:30 a.m. to 5 p.m. with the exception of final exams. Tests need to be booked at least 5 days prior to testing. Students who neglect to make a test reservation in advance may forfeit their ability to take the test in ESS.

Tests should be dropped off in ESS, Sears 470. Tests may be placed in a sealed and signed envelope and brought to ESS by the student taking the test. Tests may also be faxed to 368-8826 or e-mailed as attachments to exams@case.edu. Prior to each test, students are responsible for reminding their professors to coordinate the test administration with ESS. When students complete their tests, the tests are sealed in an envelope with an ESS staff signature across the seal. The student then returns the test to the professor unless the professor specifies that he or she will pick it up at Sears 470. Tests are kept in a locked filing cabinet while in ESS.

Referrals

If a student requests testing or any other accommodation from the professor and does not provide a memo from the office of Disability Resources, the professor should refer that student to Disability Resources. Professors are under no obligation to meet an accommodation request that has not been made by the office of Disability Resources.
