Fill this form out if your chapter would like to Adopt a Program for the academic year. This form needs to be filled out again each year. Please note that this needs to be approved by the VPs of Service for IFC and PHC as well. |
| Greek Sponsored Community Service Form |
Fill this form out to see if your community service event or program is eligible to be sponsored by Greek Life. If it is, chapters may get service hours for particiapting in this event with less than 4 people. The VPs of Service will be approving these. |

Students who want to be an active member of a fraternity complete this grade release form and submit it to a fraternity chapter. It authorizes the release of their educational records to determine academic eligibility. PDF |
| New Member - Greek Life Scholarship Form |
First-year students at Case can apply for scholarships that are available from Interfraternity Congress and Panhellenic Council. This completed form must be attached to the scholarship application prior to submission. PDF |
