Temporary ID Cards - Important Information

On July 1, 2008, the Student Medical Plan year will begin for students who are enrolled at Case Western Reserve School of Medicine, Case Western Reserve School of Dental Medicine programs (DEDMS, DORMS, DPDMS, DPEMS), and the Cleveland Clinic Lerner College of Medicine.

Need an Aetna ID card?

Visit the Aetna website to register and print a temporary ID card. You can also use this link to review claim information and other details.

Need a prescription card?

Prescription cards should be presented for medical services and will be available to print on 7/1/08. Students can download the card and print it from the Envision Rx website. You will need to know your Case network ID to print your card.

A new medical plan is available to Case students and their dependents. Our new 2008-2009 Aetna plan applies to all Case students, as well as those attending Case Western Reserve University School of Medicine, Case Western Reserve University School of Dental Medicine (MSD), and Cleveland Clinic Lerner College of Medicine.
