Optional Dependent Medical Plan
We are pleased to offer the Case Western Reserve University 2007-2008 Optional Dependent Medical Plan. Our goal is to provide comprehensive medical coverage at an affordable cost to the families of students covered by the Student Medical Plan.

The Case Western Reserve University Dependent Medical Plan offers basic and major medical coverage for services medically necessary to treat a sickness or injury. The plan also provides a prescription drug benefit.
The Dependent Medical Plan is not automatic. You must enroll eligible family members.

Important Definitions
Injury
Means bodily damages:
- caused directly and independently of all other causes by an accident; and,
- which results in loss covered by the plan.
Sickness
Means illness or disease for which treatment is received while the person is covered under this plan.
Disability
Means either a sickness or injury.
Pre-existing Condition
Means a Sickness or Injury for which medical advice, diagnosis, treatment, services or supplies were
rendered within the 12 months prior to the effective date of coverage.
Routine Care
Means any medical service that is performed for preventative purposes or performed in the absence of a
specific diagnosis.
Plan Year
Means fiscal year as described under period of coverage above.
