Dependent Medical Plan Enrollment

The University strongly recommends that dependents of students purchase some kind of insurance coverage whether it is the optional Case Western Reserve University Dependent Medical Plan or another policy. International dependents need to be aware of the requirements for insurance coverage based on their visa status. It is the responsibility of the student/dependent to ensure that their medical coverage meets the requirements set forth by their visa status.

In order to be eligible for optional Dependent coverage, you must be the spouse/domestic partner of a student who has purchased the Case Western Reserve University Student Medical Plan and you must be residing with your spouse/domestic partner. Dependent children are also eligible for coverage if they are under 19 years of age and residing with the covered student.

Enrollment forms are available at the University Health Service. An additional affidavit is necessary for the enrollment of a domestic partner. Newborns must be enrolled within 30 days of birth and residing with the covered student.
