How do I sign up for the Optional Dependent Plan?
The eligibility requirements to obtain the Optional Dependent coverage are that you must be the spouse/domestic partner of a student who has purchased the Case Western Reserve University Student Medical Plan and you must be residing with your spouse/domestic partner. Dependent children are also eligible for coverage if they are under 19 years of age and residing with the covered student.
Brochures with enrollment forms are available from University Health Service. Completed enrollment forms along with payment of premium are sent to University Health Service directly.
What does the Optional Dependent Medical Plan cover?
The Optional Dependent Medical Plan provides some coverage for most medical and mental health services. The Plan does not provide 100% coverage for all services. A copay will need to be satisfied by the covered individual. There are limitations and exclusions to the Plan. We suggest that you take the time to read about the plan carefully and understand the coverage terms. If you have any questions or need clarification about the coverages offered, call the University Health Service at 216.368.2450.
Are the services rendered for pregnancy covered?
Pregnancy is considered a sickness under the Optional Dependent Plan and is covered per the benefit guidelines outlined. Pre-existing sicknesses have different coverage limits than non pre-existing sicknesses. Please take the time to read the brochure and call the University Health Service at 216.368.2450 with specific questions regarding pregnancy coverage.
As a dependent of a student, may I use the University Health Service as my primary care facility?
Dependents are not eligible to use the services offered by the University Health Service. If covered under the Optional Dependent Medical Plan, participants are able to use the provider of their choice in the Cleveland area or outside the Cleveland area.
Because I am not the student, am I required to have insurance coverage?
The University strongly recommends that dependents of students purchase some kind of insurance coverage whether it is the Case Western Reserve University Optional Dependent Medical Plan or another policy. International dependents need to be aware of the requirements for insurance coverage based on their visa status. It is the responsibility of the student/dependent to ensure that their medical coverage meets the requirements set forth by their visa status.
Does the Optional Dependent Plan cover me if I am outside the state of Ohio?
Yes! If you need to seek medical care outside the state of Ohio, the Plan will still cover the expenses per the benefit limits outlined. To locate network providers out of state, refer to the
Beech Street Network
or call 800.877.1666. If you are outside the continental U.S. and need to seek medical care, you should pay the bill and process your claim directly with Wells Fargo Third Party Administrators. Please note: International billing/paperwork should be translated into English and paid for with American currency prior to submission for reimbursement.
I received a bill for services rendered. How do I receive reimbursement from Wells Fargo?
Pick up a claim form from the University Health Service, or call us at 216.368.2450 to have one mailed to you. Complete it per the instructions noted on the form and mail it along with your bill(s) to Wells Fargo. A separate claim form should be completed for each condition. If you need help in completing the form, please contact University Health Service at 216.368.2450.
I sent a claim to Wells Fargo and have not received anything from them telling me the status of the claim. What do I do now?
Please allow 4 weeks for processing your claim. After 4 weeks call Wells Fargo at 800.624.8605 to find out the status of your claim. If you are not able to get the information you need, please call the University Health Service at 216.368.2450, explain your situation, and we will be happy to contact Wells Fargo directly to investigate your claim.
