Student Medical Plan

We are pleased to offer the Case Western Reserve University 2006-2007 Student Medical Plan. Our goal is to provide comprehensive medical coverage at an affordable cost.

The Case Student Medical Plan offers basic and major schedule of benefits for services medically necessary to treat a sickness or injury. The plan also provides a prescription drug benefit.

The insurance plan is automatic, but optional, for students enrolled in at least one credit hour at Case. For those with other insurance coverage who elect to waive the Student Medical Plan, it is necessary that you complete a Student Medical Plan online waiver for each semester you do not need coverage. Failure to waive the coverage by the deadline each semester will result in your automatic enrollment in the plan.

If you have missed the waiver deadline, you MUST file an appeal.  Please complete the Appeal Form , in Adobe Acrobat format and mail to the University Health Service 10900 Euclid Ave. Cleveland, Oh 44106-4901 or Fax it to 216-368-8530.
