Student Medical Plan Enrollment
Case students enrolled in one or more credit hours are automatically enrolled
in the Case Student Medical Plan. You do not need to take any action to be
enrolled in the plan.
You may also be eligible for coverage if you are a resident of university
housing with a direct affiliation to a university-sponsored program, or are
associated with other special programs at the university.
Students of The Cleveland Institute of Art and The Cleveland Institute of Music registered for at least one credit
hour are also eligible. See your school for enrollment details.
The medical plan fee is $550.00 per semester for the 2006-2007 academic year.
Coverage for the fall semester begins on August 1, 2006 at 12:01 a.m. and ends
at 11:59 p.m. on January 16, 2007. Spring semester coverage runs from January
17, 2006 at 12:01 a.m. through 11:59 p.m. on July 31, 2007. Coverage dates for
students of the Case School of Medicine, the Cleveland Clinic Lerner College of
Medicine, and the Case School of Dental Medicine, MSD Program are: Fall Semester
- July 1, 2006 - January 16, 2007 and Spring Semester - January 17, 2007 - June 30,
2007. If a student registers after September 8, 2006 for the fall semester or
January 26, 2007 for the spring semester, the medical plan will become effective
on the date the student registers (not on the effective date of registration).

Students enrolled in the medical plan starting fall semester will receive an I.D.
card and Plan Information in September. The I.D. card provides important information
for students and medical providers by listing addresses to submit claims, telephone
numbers for claims inquiries. You should print a
student medical plan card until you receive one by U.S. mail. Students beginning
coverage for the first time starting spring semester should contact the University Health
Service for an I.D. card.
I am no longer covered under my parents/spouse insurance plan, can I join the Case Medical Plan?
Yes, you need to show proof of loss of coverage under the other policy. A letter from the
insurance carrier explaining the reason for termination of coverage is sufficient.
Stop by University Health Service to complete the enrollment form and pay a pro-rated fee.
Coverage will begin on the date the University Health Service receives payment.
Students must apply for coverage with the Case Student Medical Plan within 30 days of
loss of coverage from their current medical insurance.
I am taking a leave of absence for one semester, may I continue the medical coverage?
You may continue the Medical Plan coverage if your leave of absence is due to a personal
medical condition. You would be responsible to make arrangements prior to the Medical
Plan begin date of the semester in which your leave will take place. Contact the Student
Medical Plan coordinator located at the University Health Service for specific details.
What happens to my medical coverage if I withdraw during the semester?
Coverage is in effect if a student withdraws after the drop/add deadline, unless full tuition and fees are reimbursed to the student.
