Health Plan Waivers
Student who have alternate medical coverage may waive the Case Medical Plan.
A waiver must be completed each semester. The online waiver option for the
fall 2007 semester is available to all Case students though September 7, 2007.
Students attending CIA or CIM must complete a waiver form at their school.
Waiver Appeals
Case students who miss a waiver deadline and wish to file an appeal may print the appeal form and return it to the University Health Service.
