Student Medical Plan
We are pleased to offer you an attractive option to meet your medical and dental needs while you are a
student at Case Western Reserve University in 2007-2008. Please note, that in response to the continued rising costs
of healthcare, there have been changes made to the Student Medical Plan for 2007-2008.

The Student Medical Plan is offered as a supplement to the excellent care available to all Case Western Reserve University students from the
University Health Service and the University Counseling Services. The Student Medical Plan is sponsored by
Case Western Reserve University. The plan extends eligibility to The Cleveland Institute of Music, The Cleveland Institute of Art, and Special Programs.
The university also offers the Optional Dependent Medical Plan for those students who wish to purchase
coverage for their dependent spouse, domestic partner and children. A separate summary plan description
is available with enrollment and payment information.
Our intent is to provide you with the opportunity to obtain efficient and effective medical coverage that
responds to your individual needs. Please help us in this regard by sharing your thoughts and suggestions.

Note: If you missed the waiver deadline of September 11, 2007, you MUST file an appeal. Please complete the appeal form and mail it to University Health Service, 10900 Euclid Ave., Cleveland, Oh 44106-4901. Or, you may fax the form to 216.368.8530.
