Mental Health and Chemical Dependency Coverage
In-Network: 80% of eligible charges are paid for by The Plan. The remaining 20% of charges are the responsibility of the student.
Out-of-Network: 60% of eligible charges are paid for by The Plan. The remaining 40% of charges are the responsibility of the student.
Outpatient Mental/Nervous Benefit Limitations: Coverage under The Plan is limited to 26 visits per Plan
Year for: diagnosis, treatment and couples counseling, legally rendered by a licensed psychiatrist, psychologist
or social worker. Such outpatient service may be provided in a physician's office, hospital outpatient
department, community mental health facility or an alcoholism or drug abuse treatment facility.
Biofeedback is covered only when referred by University Counseling Services.
