$10 copay, 1 per Plan Year.

20% discount when a complete pair of glasses is purchased.

20% discount when a complete pair of glasses is purchased.
- Contacts
15% discount off the contact lens fitting and evaluation exam. This exam is in addition to the vision
exam to ensure proper fit of contacts.

Participants are encouraged to visit a VSP doctor. Failure to do so will result in a lesser benefit. Discounts
on lenses, frames and contact lens exams only apply when obtained through a VSP doctor.
About VSP Eyecare
VSP network doctors are in medical offices and shopping centers throughout the Cleveland area. They have a
large frame and contact lens selection and offer evening and weekend hours and accept drop-ins.
Appointments
Choose a VSP doctor online or call 800.877.7195. Make an appointment. Inform the doctor of VSP coverage. No ID cards or claim forms required.
Out of Network Coverage
Participants who use a provider not in the VSP network are required to pay the provider in full at the time of
appointment, and submit a claim to VSP for partial reimbursement. The maximum reimbursement is $35
per Plan Year.
