2008-2009 Student Medical Plan
We are pleased to offer you an attractive option to meet your medical, dental, and vision needs while you
are a student at Case Western Reserve University. Please note, that in response to the continued rising
costs of healthcare, there have been changes made to the Student Medical Plan for 2008-2009. The
Student Medical Plan is offered as a supplement to the excellent care available to all Case Western
Reserve University students from University Health Service and University Counseling Services.

The University also offers the Optional Dependent Medical Plan for those students who wish to purchase
coverage for their dependent spouse, domestic partner, and children. A separate summary plan description is
available with enrollment and payment information.
Our intent is to provide you with the opportunity to obtain efficient and effective medical coverage that
