Mental Health Coverage
In-Network: 80% of eligible charges are paid for by the Plan; the remaining 20% of charges are the responsibility of the student.
Out-of-Network: 60% of eligible charges are paid for by the Plan; the remaining 40% of charges are the responsibility of the student.
Outpatient Mental/Nervous Benefit Limitations: Coverage under the Plan is limited to 26 visits per Plan
Year for: diagnosis, treatment and couples counseling, legally rendered by a licensed psychiatrist, psychologist
or social worker. Such outpatient service may be provided in a physician's office, hospital outpatient
department, community mental health facility, or an alcoholism or drug abuse treatment facility.
